New Patients
Stop by one of our practices to complete a “Records Release Form.” This gives us permission to request your child’s/children’s records from their previous Pediatrician(s) office.
Once we receive your child’s/children’s records from the previous pediatrician(s), our providers will review them to ensure all your child’s/children’s needs are met here at our practice.
Then we will contact you to set up all necessary appointments.
New Born Patients
When you deliver your baby, tell the hospital staff that your baby will be a patient of Dr. Alappan from Columbus Pediatrics GA. You or the hospital staff will contact our office 3-5 days after discharge to schedule your baby’s newborn first visit.
Please bring all Discharge paperwork from the hospital to your Newborn’s First visit.
